De problemen verdwijnen meestal niet helemaal en\u202fkomen\u202fvroeg of laat (bij stress of een verhoogde infectiedruk)\u202fterug.\u202f\u202fBovendien zorgen de subklinische of \u2018verborgen\u2019 gevallen ervoor dat pneumonie een blijvende uitdaging vormt.<\/p>\n<h3>De\u202fdetectie van pneumonie blijft een uitdaging\u202f<\/h3>\n<p>De diagnostiek van pneumonie bij kalveren start in de praktijk met een klinisch onderzoek.\u202fBij het\u202fklinisch onderzoek, wordt\u202faandacht besteed aan symptomen zoals hoesten, neusuitvloeiing, verhoogde ademhalingsfrequentie, koorts, verminderde eetlust en algemene conditie.\u202fKlinische scorekaarten, zoals de \u2018Wisconsin\u202fRespiratory\u202fScoring Chart\u2019,\u202fkunnen hierbij van hulp zijn.<\/p>\n<p>De valkuil bij het gebruik van deze kaarten is dat ze afhankelijk zijn van zichtbare klinische symptomen, waarmee subklinische gevallen gemist worden\u202f(kan oplopen tot 30%).\u202fPas wanneer de klinische bevindingen daar aanleiding toe geven, worden aanvullende onderzoeken ingezet om de etiologische agentia op te sporen.\u202f<\/p>\n<p>Voor de diagnostiek van infecties van de lagere luchtwegen wordt vaak een non-endoscopische bronchoalveolaire lavage (nBAL\/longspoeling) uitgevoerd. Met behulp van deze\u202fnBAL\u202fkrijgen we informatie over de aanwezigheid van ontstekingscellen\u202fen\u202fziekteverwekkers\u202fals\u202fbacteri\u00ebn, virussen en schimmels.\u202f<\/p>\n<p>Minder invasief, maar\u202fook\u202fnuttig, is het nemen van\u202fdiep\u202fnasopharyngeale\u202fswabs\u202f(DNPS) voor\u202fmicrobiologisch onderzoek.\u202fStudies hebben aangetoond dat\u202fdiep\u202fnasopharyngeale\u202fswabs en\u202flongspoelingen\u202fdezelfde capaciteit hebben om\u202fde\u202fvoornaamste bacteri\u00eble veroorzakers als\u202fMannheimia\u202fhaemolytica,\u202fPasteurella \u202fmultocida\u202fen Mycoplasma\u202fbovis\u202fop te sporen.\u202f <em>Mannheimia haemolytica<\/em>, <em>Pasteurella multocida<\/em>, and <em>Mycoplasma bovis<\/em>.<\/p>\n<p>Voor virale infecties ligt het iets anders. Virale infecties als \u202fBovine\u202frespiratory\u202fsyncytial\u202fvirus\u202f(BRSV) en\u202fParainfluenza \u20133 virus (PI3) kunnen gemakkelijker worden opgespoord met behulp van een longspoeling, terwijl bovine coronavirus (BCoV) gemakkelijker zal kunnen worden gedetecteerd uit diep nasopharyngeale swabs.<\/p>\n<p>Aangezien het in de meeste gevallen onduidelijk is welke ziekteverwekkers primair zijn, lijkt een combinatie van zowel DNPS als nBAL monsters relevant om de gevoeligheid van de diagnostische procedure te vergroten.\u202f\u202f<\/p>\n<h3>Longechografie en zijn additionele waarde\u202f<\/h3>\n<p>In de wetenschappelijk literatuur wordt meer en meer\u202fook het uitvoeren van\u202flongechografie aanbevolen.<\/p>\n<p>Met echografie kunnen afwijkingen zoals longconsolidaties, pleurale veranderingen en abcessen in beeld worden gebracht, zelfs wanneer klinische symptomen beperkt aanwezig zijn.\u202f\u202fHierdoor ontstaat een completer beeld van zowel de oorzaak als de ernst en uitgebreidheid van de longschade.<\/p>\n<p>Bovendien kan longechografie aan het dier worden uitgevoerd zonder sedatie en levert het onmiddellijk resultaten op.
